The privacy principles and procedures set forth in this Website Privacy Statement ("Privacy Statement") are intended to tell users how this information is collected, used, and distributed via the Website and applies solely to the information that we collect through the Website. This Privacy Statement does not address personal information that you provide to us in other contexts (for examples, through a business or investment relationship not handled through the Website).
Acceptance of Privacy Statement
By using the Website, you signify your acceptance of this Privacy Statement. If you do not agree to the terms and conditions of this Privacy Statement, then you should not use this Website. Your continued use of this Website following the posting of changes to the terms and conditions of this Privacy Statement means that you accept those changes.
What This Privacy Statement Covers
This Privacy Statement covers how Onegevity Health treats personally identifiable information ("PII") that Onegevity Health collects and receives through the Website. PII can include, but is not limited to, your full name, e-mail address, Internet Protocol address, login name, and, for use by a health-care practitioner, your professional licensing information.
PII Collection and Use
How Personally Identifiable Information Provided By You May Be Used. Except as described in this Privacy Statement, Onegevity Health only collects your PII through the Website when you choose to provide such information, such as when you create your Onegevity Health account. To create your Onegevity Health account, we require basic information from you, including your name, address, and a valid e-mail account. You have the option to provide additional information, including your professional licensing information if you are a health-care practitioner. Onegevity Health may use your PII:
- To create your Onegevity Health account
- To identify you on the Website
- To communicate with you
- To share information and news relevant to the Services
- To address your requests for information or services
- To respond to your inquiries
- To send you administrative communications
Surveys. From time-to-time the Website may request information from you via surveys. Participation in these surveys is completely voluntary and you may choose whether or not to participate and therefore disclose the requested information. Information requested could include information about your organization’s practices or your opinions. Survey information will be used for purposes of monitoring or improving the content, use, and satisfaction of this Website.
Onegevity Health will not sell, rent, license, or trade your PII with third parties for their own direct marketing use unless we receive your express consent to do so. Unless you give us permission to do so, Onegevity Health will not share your PII other than as specified in this Privacy Statement.
Without obtaining your express consent, Onegevity Health will not sell, rent, license trade, or trade your PII to any third party for their own direct marketing use.
Disclosures to Third Parties Assisting in Our Operations. Onegevity Health may disclose PII to a third party that works with, or on behalf of, Onegevity Health to provide resources or services (for example, packaging, sending, or delivering a purchase or information to you, or clearing a credit card transaction). These companies may use your PII to assist Onegevity Health in performing our operations. These companies do not, however, have any independent right to share this information.
Disclosures Under Special Circumstances. Onegevity Health might be required to disclose your PII to respond to a subpoena, a court order, legal process, or a governmental regulation, or to establish or exercise our legal rights or defend against legal claims. In addition, Onegevity Health might disclose your PII to a third party that is assisting us in conducting an investigation about unauthorized activity that constitutes a violation of the policies for using the Website, or when circumstances dictate the need to protect or defend our rights or property. We may also disclose your PII if we believe it is necessary to share that information to investigate, prevent, or take action regarding illegal activities, suspected fraud, situations involving potential threats to the physical safety of any person, or as otherwise required by law.
Business Transfers. We may share your PII with other business entities in connection with the sale, assignment, merger, or other transfer of all or a portion of Onegevity Health’s business to such business entity. We will require any such successor business entity to honor the terms of this Privacy Statement.
Automatically Collected Information and Anonymous Information
Each time a visitor comes to the Website, Onegevity Health collects some information to improve the overall quality of the visitor’s online experience.
Anonymized and Aggregated Statistical Data. Onegevity Health collects aggregate queries for internal reporting. Onegevity Health also counts, tracks, and aggregates the activity of visitors into Onegevity Health’s analysis of general traffic flow at the Website. Onegevity Health may also remove personal identifiers from PII and maintain it in anonymous and aggregate form that may later be combined with other information to generate anonymized and aggregated statistical information. Onegevity Health may share, publicly or privately, anonymized and aggregated statistical information from the Website. If Onegevity Health shares anonymized and aggregated statistical information, then it will do so in a manner that no specific individual can be identified by its use. Anonymized and aggregated statistical information may be displayed on the Website as part of collaborative learning.
Web Server Logs and IP Addresses. An Internet Protocol ("IP") address is a number that automatically identifies the computer/device that you have used to access the Internet. The IP address enables our server to send you the webpages that you want to visit. The IP address may disclose the server owned by your Internet Service Provider. Onegevity Health may use IP addresses to conduct website analyses and performance reviews and to administer the Website.
In addition to using cookies, the Website may employ "pixel tracking," which is a common process used in connection with advertisements and/or internal tracking. Pixel tracking involves the use of pixel tags, an invisible tag that is placed on various pages of a website that tracks a user's activity. Pixel tags are not visible to users and consist of a few lines of computer code. Pixel tracking measures the effectiveness of advertisements by compiling aggregate and specific usage statistics. Onegevity Health may access pixel tags to identify the activities and interests of users that better enables us to match products, services, and offers to the user's interests and needs. For example, if a user visits the Website as a result of an advertisement on a third-party website, then the pixel tag will allow the advertiser to track that its advertisement brought the user to the Website. If a user visits the Website or other online system operated by Onegevity Health, and the user is linked to another website, then Onegevity Health also is able to determine that the user was sent to and/or interacted with a third-party website. This data is collected for use in Onegevity Health’s marketing and research.
If you wish to opt-out of Google Analytics entirely when visiting the Website, or interest-based advertising, then visit the Network Advertising Initiative at: http://www.networkadvertising.org/choices. Or if you wish to specifically choose not to send information to Google Analytics about your visit to the Website, then you may do so by going to: https://tools.google.com/dlpage/gaoptout.
Onegevity Health regularly implements industry-recommended technological and operational security measures it believes are reasonably designed to protect PII from unauthorized access, loss, misuse, alteration, or unintentional destruction. Although no security measure can absolutely guarantee against unauthorized uses, Onegevity Health regularly reviews and updates its website security measures for the purpose of providing appropriate security measures for PII. Onegevity Health provides no guarantee, however, that your PII will remain secure.
Response to "Do Not Track" Signals
Some Internet browsers include the ability to transmit "Do Not Track" signals. Because uniform standards for "Do Not Track" signals have not yet been adopted, Onegevity Health does not process or respond to "Do Not Track" signals.
California Civil Code Section 1798.83 permits users of our Website who are California residents to request certain information regarding our disclosure of PII to third parties for their direct marketing purposes. To make such a request, you may contact us by e-mail at email@example.com.

The Website and the services on which the Website is hosted are located in the United States and are intended only to be used by individuals in the United States. PII you submit using the Services will be transferred to and stored in the United States. By visiting the Website and submitting PII, you authorize this transfer.

Onegevity Health will not retain your PII longer than is necessary for the purpose it was collected.
Onegevity Health's policy is that children under the age of 18 should not provide any PII to the Website without a parent's or guardian's consent. Onegevity Health encourages parents and guardians to instruct children about the safe and responsible use of PII on the Internet. Onegevity Health will not knowingly collect, use, or disclose to any third party any PII collected from children under the age of 18.
Onegevity Health is the sole owner of the information collected on the Website. We only have access to and collect information that you voluntarily give us via the enrollment process, through e-mail, or other direct contact from you.
